Avery 5160: label 2.625 in by 1 in, top 0.5 in, side 0.1875 in, column gap 0.125 in, row gap 0 in. The preview is scaled. Print the PDF at 100 percent.

The outlines are the label positions. Load rows to fill them.

Address lists are the kind of file you do not want sitting on someone else's server. LabelCSV reads the spreadsheet here and writes the PDF here.
